Key features of the course include: - This presentation will cover: The hospital discharge process can be stressful and frustrating to patients and their families alike. When discharges are poorly planned, families are coerced to make critical care decisions at the last minute, placing immense pressure on families to make critical decisions without sufficient time or information for choices to be informed. This ACES presentation will identify the key role and tenants of effective patient/family advocacy when patients are discharged home or to a Skilled Nursing Facility/Rehab Center. Each scenario is fraught with challenges that must be understood and addressed proactively. We hope to provide clear and concrete actions advocates can take to stay informed and actively involved in the Discharge Planning process to ensure a safe discharge. - This course does not offer CE credits.
